NOTICE IS HEREBY GIVEN that the Applicant is requesting that the Commission grant an exception to OCC-OGR Rule No. 165:10-5-6 insofar as same requires a mechanical integrity test of the Milroy Deese Unit 14-4 well as an injection well by allowing the Milroy Deese Unit 14-4 Well to inject under the authority of the UIC Permit to issue as No. 1502370051 as long as said well passes a mechanical integrity test to a pressure of 200 PSI, said test to be run annually instead of every five years as required by Commission rules, and possibly making the Order effective on some date prior to the entry of the final order. NOTICE IS FURTHER GIVEN that the Applicant is requesting the following special relief: Possibly providing that said Order be made effective on some date prior to the entry of the final order.
