LEGAL NOTICE In the Court of Common Pleas of Lawrence County, Pennsylvania: No. 7009 of 2014 M.d., In re Petition of Rylee Ashton Brewer for change of name to Rylee Ashton Fritzley. To all persons interested: Notice is hereby given that an order of Court authorized the filing of said petition and fixed the 22nd day of January 2015 at 1:30 o'clock p.m. as the time and Courtroom No. 1 in the Lawrence County Government Center as the place for a hearing upon said petition, when and where all persons may show cause, if any, they may have, why said name should not be changed as prayed for. Legal: December 6, 2014 POLICY STATEMENT Non-Discrimination Title VI This facility has agreed to comply with the provisions of Title VI of the Civil Rights Act of 1964 and the Pennsylvania Human Relations Act of 1995 and all requirements imposed pursuant thereto, to the end that no person shall on the grounds of age, race, color, sex, sexual preference, national origin, handicap, ancestry, religious creed, or source of payment, to excluded from participation in, be denied benefits of, or otherwise subjected to discrimination in the provision of any care of service. This non-discriminatory policy of the institution applies to patients, physicians, and all responsible employees. Avalon Nursing Center has also agreed to comply with the following major laws and their provisions. Section 504 of the Rehabilitation Act of 1973 and the Age discrimination Act of 1975, as amended. Specifically, the above includes (but is not limited to) the following activities, services, or actions: 1. Inpatient care and any contracted services for residents shall be provided regard to the grounds set forth above. 2. All residents shall be assigned to rooms, and sections in accordance with their medical needs. 3. Residents shall not be asked whether they are willing or desire to share a room with a person of another race or religion. 4. Transfer of residents from rooms assigned or selected or both, shall not be made for other than valid medical reasons. 5. Discharged as appropriated residents shall be referred only to those Hospitals, Skilled Nursing Care Facilities, Intermediate Care Facilities, or Personal Care Facilities, which are not known to Avalon's to be in non-compliance with the provisions of the Pennsylvania Human Relations Act (43 P.S. Sec. 9510953). Avalon Nursing Center will report immediately to the Compliance Office of the Department of Health all known instances of discriminatory practice experienced by residents referred by the Avalon Nursing Center.
