No. FA-2014-103 NOTICE OF HEARING TO TERMINATE PARENTAL RIGHTS AND TO ALLOW ADOPTION WITHOUT CONSENT OF ALLEGED FATHER THE STATE OF OKLAHOMA, TO: Kevin Dean Mastin 10701 Olde Tuscany Road Oklahoma City, OK 73173 On the 2nd day of December, 2014, an Application was filed in this Court by the adoptive parents seeking an order of this Court to permit the adoption of child K.H.M. without the consent of the alleged father and to terminate rights of alleged father. The minor child, K.H.M., was born on the 12th day of July, 2005 to Lindsay Nicole Medlock, now Welch, and Kevin Dean Mastin at Oklahoma City, Oklahoma. A hearing on said Application is set for the 2nd day of February, 2015, st the hour of 9:15 oclock, a.m. in the District Courtroom in the District Courthouse of Cleveland County, Norman, Oklahoma, before the Honorable Stephen W. Bonner.
